Aknowledgment:

• Some products may be restricted in your area ; we may require a confidential medical questionaire to issue products by prescription.
• If presently you are not under the care of a Doctor, please fill out the Medical QUESTIONNAIRE (important to list all medications you are on) and one of our Physicians will review your health history.
• If necessary; recommendation on our products and dosage will be made; should any of the products you are taking is not listed on our recommended dosage.
• If medically suitable ; they will provide a prescription to accompany each shipment. For this service we require a one time consultation fee $100.00 to be paid with your first order only and a prescription fee of $20.00 for each order after that. This service is provided for your safety.
• We plan to provide telephone consultation service with your Physician in the very near future. Estimated consultation fee is $10.00/minute via 900# phone lines. (You must quote your Life Energy chart number)
• All prices are in Canadian Dollars and subject to change without notice. Your credit card will be charged on the day the order is shipped.
